In this role, you’ll provide high-quality, hands-on care to seniors in post-acute and senior living facilities across the Midwest region. You’ll make a meaningful difference by supporting residents during key moments in their health journey, while also building strong relationships with care teams and facility staff. This is an exciting opportunity for a compassionate clinician who values autonomy, variety, and the chance to grow within an innovative, patient-centered organization.

Essential Duties & Responsibilities

  • Provide direct care for seniors in partner facilities, including skilled nursing, long-term care, assisted living, independent living, and memory care.

  • Travel regionally (4 days per week) to bring consistent, high-quality care where it’s needed most.

  • Conduct assessments, manage chronic and acute conditions, and ensure continuity of care.

  • Collaborate with facility leaders, nursing staff, and Curana’s multidisciplinary care team to improve outcomes and address resident needs.

  • Serve as an ambassador for Curana Health, fostering strong relationships and supporting smooth transitions to permanent providers.

  • Maintain accurate, timely documentation and billing in alignment with policy and compliance standards

  • Flexibility & Autonomy: Enjoy the independence of practice with the backing of a seasoned support team.

  • Professional Support: Access to a dedicated administrative and clinical team to help you focus on patient care.

  • Innovative Model: Be part of a forward-thinking approach to senior healthcare, supported by data-driven tools and resources.

  • Travel Support: Covered travel expenses so you can focus on care, not logistics.

  • 401(k) with company match

  • Paid Time Off (PTO)

  • License reimbursement

  • CME reimbursement

  • Company-paid short/long-term disability

Qualifications

  • NP degree with unrestricted licensure (ability to obtain additional state licenses within 30 days).

  • Easy access to a major airport for frequent regional travel.

  • Compassionate, team-oriented, and motivated to make a difference in senior healthcare.

This role requires travel to and from one or more assigned facilities using a personal vehicle. A valid driver’s license, current auto insurance, and an acceptable driving record are required.
